When creating a client custom fillable forms, add the SSN to the demographic block. Clients currently have to add the SSN in a separate field if they wish to include this in the forms.

For the immunization report that we can print off for clients, if we do a TB skin test or blood lead, it would really benefit if it would auto-populate like the immunizations so when we print them off the client has record of them.
