Allow Medication and Allergy Reports to include option for "include inactive medications"

When running the form style Medications and Allergies report, when it comes up with the check boxes to include physician signature line and/or include electronic signature have another checkbox that says include inactive medications but by default the report would only include active medications. That way they would not have to specify certain pages to print in order to avoid printing inactive medications.
