Provide a box for birth-weight under Child Vitals. If the child is coming in for Well Child Check ups we would like to have a place for the birth-weight that would also show up on the growth chart. Would also like a box to indicate that a weight was recorded by another provider.

This should also be a field that carries through activities like the due date and delivery date do, so that the nurse can compare as the child is seen.
