We currently have a aide orientation sheet we used when aides have new assignments. It would be ideal to be able to use the assignment sheet to fill in other features such as schedule (we usually put in frequency and lime allowed if applicable). I have attached our current sheet. Wondering if there was a way to have more options available. Could eliminate our current sheet. Also, could not having diagnoses listed be an option? Some clients really do not like addressing their diagnoses especially if it includes mental health topics.
