Show Pay Source phone number on Subscriber Tab

Add the pay source phone number to pay source setup and then display that on the subscriber tab for that pay source.

On the Subscriber tab, add these fields at the bottom of the popup screen:
- Payer ID - text field
- Claim filing indicator - dropdown that contains the same values as the Claim filing indicator field in Pay Sources
